Public summary

FaceofMind may make a Business Associate Agreement available to eligible covered entities and business associates before HIPAA-regulated workflows go live. This public page is an informational summary only and does not replace a signed BAA.

Important note

No customer should rely on this page as proof of an executed BAA. If your organization needs HIPAA contractual coverage, do not send PHI into production workflows until contracting, technical review, and onboarding are complete.

What a BAA request usually covers

  • Permitted uses and disclosures of protected health information (PHI) within the contracted service scope.
  • Safeguards for confidentiality, integrity, and availability of covered data.
  • Breach or incident cooperation, notification support, and subcontractor handling where applicable.
  • Return, deletion, or continued protection of PHI at termination, subject to legal and operational requirements.
